| Your contact information (name, company, address, phone #, email address, and billing information); |
Claimant's name and any other identifying information (e.g. claim #, SS#, DOB)
| Location for IME: please choose from one of the following locations, depending upon where the claimant |
either lives, works, or currently resides: Madison, Tomah, Eau Claire, Wausau, Green Bay, Milwaukee.

Dr. Goodman will contact you either by email or phone to confirm the appointment.
Note: Dr. Goodman also works with a variety of IME vendor/partners. He prefers direct
referral (i.e. non-vendor) because of financial considerations. However, if you prefer to contact him
through an IME vendor, you may do so. Popular IME vendors he partners with include: Exam Works
