schedule. If you would like to schedule an IME fill out the
online scheduling
form or simply email
drdave@drdavegoodman.com the name of the individual, the location and
date.
Please note: All IME examinations are preformed at medical facilities throughout the state. Additional locations and dates may be available on special request. November 13: Tomah and EauClaire November 14: Wausau and Green Bay November 17: Delafield and Madison Appointments are available in all locations unless otherwise indicated.
